Bathing
Cues to initiate showering/ bathing and set up of supplies. Assistance with safe care moving in and out of the shower.
Intermittent hands-on assistance (washing hard to reach areas, etc.) with showering. Set up of supplies. Assistance with safe care moving in and out of the shower. May be resistant to care.
Continual hands-on assistance with showering and bathing (undressing, washing, drying, and redressing). Set up of supplies. Assistance with safe care moving in and out of the shower. May be resistant to care.
Extensive, hands-on assistance with showering and bathing at a frequency required to maintain a proper level of personal hygiene. May require more than one person to assist in completing task. May be resistant to care.
Personal
Grooming
Cues to initiate and complete grooming. May include set up of grooming materials. Hands on assistance with shaving and denture care, if required.
Intermittent hands-on assistance with dental care and hair styling. Set up of supplies. Possible task segmentation to facilitate completion of personal hygiene activities. May be resistant to care.
Continual hands-on assistance to complete personal grooming. Set up of supplies. Requires direct instruction to facilitate completion of personal hygiene activities. May be resistant to care.
Extensive, hands-on assistance up to and including the completion of grooming. May be resistant to care.
Dressing
Cues to initiate dressing. May have to offer and encourage proper selection of clothing choices.
Intermittent assistance with dressing, laying out clothing and cueing to get dressed or undressed. Requires task segmentation to facilitate completion of task. May be resistant to care.
Continual hands-on assistance with dressing and undressing. Requires direct instruction in order to complete task. May be resistant to care.
Extensive hands-on assistance to complete dressing task. Resistant to care or does not participate in completion of dressing activities.
Mobility
No mobility impairments. May need reminders to take occasional rest periods.
May use assistive device for ambulation (cane, walker, etc.). Requires transfer assistance and/ or intermittent assistance and supervision. May need to monitor for perceptual difficulties.
Uses assistive device for ambulation (cane, walker, etc.). Requires continual assistance and supervision with mobility. Monitoring and interventions for perceptual difficulties.
Hands-on assistance with ambulation and assistance with wheelchair for longer distances. May require one or more people to assist.
Toileting
No physical assistance required. Directional cues and reminders to use the bathroom. May need cues for bathroom location.
Intermittent assistance with continence management. May require reminders and limited physical assistance to maintain continence. May require limited assistance in the bathroom. May be resistant to care.
Continual assistance with incontinence management. Complete incontinence is fairly easy to manage. Requires continual, hands-on assistance in the bathroom. May be resistant to care.

Eating
Assistance making menu choices. Cues and reminders of meal times and locations. Monthly weight monitoring.
Assistance to and from the dining area. Intermittent interactive motivation to eat. Requires intermittent assistance with cutting food. Monthly weight monitoring.
Assistance to and from the dining area. One-on-one interactive motivation to eat. Continual assistance with meals such as cutting food and using condiments. Possible modifications to increase meal consumption and beverage intake. Monthly weight monitoring.
Assistance to and from the dining area. Extensive hand-over-hand assistance with completion of the meal. May require meal supplement to maintain adequate calorie intake. Monthly weight monitoring.
